✔️ Side Effects: Telma 40
- Common in Hypertensive Patients:
- Sinus pain & congestion (sinusitis).
- Back pain.
- Diarrhea.
- Common in Cardiovascular Risk Reduction Patients:
- Intermittent claudication (leg pain due to poor circulation).
- Skin ulcers.
✔️ Pharmacology:
- Telmisartan:
- A non-peptide angiotensin II receptor antagonist.
- Blocks angiotensin II from binding to AT1 receptors, preventing:
- Vasoconstriction
- Aldosterone secretion
- Cardiac stimulation
- Sodium reabsorption in kidneys
- Selectively binds to AT1 receptors (>3,000 times more than AT2 receptors).
- Does not inhibit angiotensin-converting enzyme (ACE), and thus does not affect bradykinin levels.

- Starting dose: 40 mg once daily.
- Dose range: 20-80 mg once daily (dose-related blood pressure response).
- Maximal effect: Achieved after 4 weeks.
- Elderly & Renal Impairment: No initial dosage adjustment is required.
- Patients on Dialysis: Monitor closely for orthostatic hypotension.
- With or without food.
- Not recommended for:
- Patients ≥75 years old
- Those with hepatic impairment
- Pediatric patients: Safety & effectiveness not established.
- Geriatric patients: No significant differences in safety & effectiveness compared to younger patients.
- Hepatic impairment: Monitor closely & up-titrate slowly.

- Symptoms:
- Hypotension, dizziness, tachycardia (fast heart rate).
- Bradycardia, increased serum creatinine, acute renal failure (due to vagal stimulation).
- Treatment: Supportive care, IV fluids, monitoring vitals..
✔️ Drug Interactions:
- NSAIDs:
- May reduce antihypertensive effects.
- Increases risk of renal impairment.
- Aliskiren:
- Avoid the use of Telmisartan in diabetic patients due to the increased risk of complications.
✔️ Contraindications:
- Hypersensitivity to Telmisartan or any component of the product
✔️ Pregnancy & Lactation:
- Pregnancy Category C (First trimester) & Category D (Second & third trimesters).
- Can cause fetal toxicity – avoid during pregnancy.
- Lactation:
- May cause adverse effects in nursing infants.
- Either discontinue nursing or discontinue the drug.

- Avoid fetal or neonatal exposure (can cause severe birth defects).
- Monitor for hypotension (especially in volume-depleted patients).
- Caution in hepatic & renal impairment.
